Anchor Homes Services

Anchor Homes is designed with the assistance of the individual, their family or legal guardian, the case manager, the behavioral clinician, BDDS service coordinators, direct care staff and others to create an individualized support plan that addresses the individual's needs and reflects their interests and wishes. Our services are also based on the individual's person-centered plan that considers the individual's long term and short term interests and needs that are reflected in their support plan. These services also include the health, safety and welfare of the individual and assist in the acquisition, improvement and retention of skills necessary to support an individual to live successfully in their own home. We have an RN on staff who assesses each client regularly and is available for emergency situations. Each of our homes has a home manager in the home 32+ hours a week to assit our clients and to provide assistance to the staff assigned to the home. The QIDP assigned to each client visits them weekly or as need. We also have a payee representative available for any clients needing this service. All of our staff have worked directly with the DD and Autistic population extensively.
